Boarding Agreement Form: Book Your Pet’s Boarding Stay

When your pet stays with River Road Animal Clinic, you can rest assured knowing that they are being loved and cared for by our experienced team. Prior to your pet’s stay with us, we ask that you please fill out the Boarding Agreement Form online, or you can print a copy to fill out and bring with you when dropping off your pet. Thank you for entrusting River Road Animal Clinic, LLC (RRAC) with your “Fur Baby”.

Boarding requirements: All canines and felines must be up to date on vaccines, including Rabies, Dhppv, Bordetella for canines and Rabies and FVRCP for felines before check in date. Official vaccination records need to be furnished for new client pets before the scheduled check in date. RRAC reserves the right to refuse to accept a pet at check in for any reason, including and without limitations, if the pet is sick, injured, in pain or it’s behavior could jeopardize the health or safety of other pets or our staff. If while boarding the pet is found to be sneezing, coughing, or having problems with urinating or defecating, RRAC reserves the right to treat the pet as needed. If there any other major issues that occur while in the care of RRAC, the owner or the emergency contact person will be contacted.

If your pet is showing signs of illness, i.e. sneezing or coughing, vomiting or lethargy at the time of check in, please inform the RRAC receptionist. An exam will be performed and your pet will receive the needed treatment.

We welcome and recommend you bring your pet’s foods with detailed feeding instructions, or RRAC will provide Hill’s Science Diet Sensitive Skin and Stomach diet. Blankets and comforters are provided for bedding. If you bring items from home, RRAC is not responsible for damage or loss.

Medication: If your pet needs to be given medications while boarding, there will be an extra $2.00 charge per day per pet.

Please bring all medicines in the original containers with the prescription label on them. If you have made any changes in medications, please notify the receptionist at check in.

Liability: Please understand that even if your pet is fully vaccinated, it can still become ill. Vaccines are not a guarantee of protection against Kennel Cough, Parvo virus, or canine influenza. If your pet becomes ill while boarding or after going home, you are responsible for all costs of treatment. You acknowledge and agree that in an unlikely event your pet is injured, you release River Road Animal Clinic, LLC, and its employees from liability for such injury. If your pet injures another pet, you will be solely responsible for any injury to the other pet(s) as well as your own pet. You also agree to release River Road Animal Clinic, LLC and the staff from any liability for such injuries including all expenses for pets involved. Please let the receptionist know if you have any questions concerning this agreement.

We accept Visa, Mastercard, Discover, and cash only. Payment is due at time of pick-up.

Hours of operation: Monday through Friday 8:00am - 12:00pm and 1:00pm - 5:30pm. Closed Saturday and Sunday and all major holidays.
